Comprehensive assessment of needs of clients over 65 years and carers. Provision of services to meet needs (including referrals to other services where necessary). The team provides sound work, physio and OT services. A nurse is also attached to the team and she deals with clients in Nursing Homes at present, though this is due to change with the introduction of a permanent care review team for Residential and

The service is provided for all clients who have a severe and enduring illness and depending on their needs they will be provided with the most appropriate follow up and care - staff within the team which is multidisaplinary in nature are trained and qualified in different areas to provide the most up to date evidence based practice. If their needs cannot be met within team then other agencies can be accessed.
